Dear Parent/Guardian, 

The Moon Area Athletic Department will offer sports physical exams for all students entering grades 7–12 on Tuesday, May 19th from 6:00 PM – 9:00 PM in the Moon Area High School Auxiliary Gym. Sports physical exams are provided by Allegheny Health Network and are $20 (cash or check). All checks should be made payable to “AHN”. 

All student-athletes must have a current physical examination dated May 1, 2026 or later in order to participate in athletics during the upcoming school year. Pages 5 and 6 of the PIAA physical form must be completed. Section 5 should be filled out by the parent/guardian prior to the exam, and Section 6 will be completed by the physician during the physical exam. School Physical Exam forms are NOT accepted for interscholastic athletic participation.

The Athletic Department will also host Concussion Baseline Testing on Wednesday, May 27th from 5:00 PM – 7:00 PM in the Moon Area High School Auxiliary Gym. 

All incoming 7th, 9th, and 11th grade student-athletes, along with any student who is new to participating in school sports, are required to complete a concussion baseline test. Concussion Baseline tests are valid for two years and are free. 

All students participating in concussion baseline testing must bring a cell phone with them to the testing session.
