The Moon Area Athletic Department will be holding Concussion Baseline Testing for students entering grades 7, 9, and 11 ONLY for the 2025-2026 school year on:

Tuesday, May 27th
5:00 PM – 7:00 PM
Moon Area High School Auxiliary Gymnasium (Use Main Entrance)

Appointments are required. Please use the following link to schedule your testing time:

Choose an available time slot. Testing will take approximately 20–30 minutes.

Important Information:

• Parents do not need to be present during testing.
• Testing is mandatory for all athletes entering grades 7, 9, and 11.
• A cell phone is required with the Sway Medical app downloaded before arrival.
• Search for "Sway Medical" in the App Store (iOS) or Google Play Store (Android).
• Download the app prior to testing.
• You will receive a testing code upon arrival.
