PAID COACHES
All paid coaches employed through the school district must complete the following clearances and forms in order to be considered for employment:
- ACT 114 FBI clearance- The Commonwealth of Pennsylvania has changed the provider for this clearance. For more information regarding the new process, click here. Follow these instructions to register for fingerprints:
- Visit https://uenroll.identogo.com/
- When prompted for a Service Code, enter 1KG6XN
- On the next screen, click "Schedule or Manage Appointment"
- Fill out all the required information. As you fill out the required information, you will be asked to search for a place to have your fingerprints captured. The locations are not the same as the old Cogent sites. You will choose a day and time, or just walk-in, to have your fingerprints captured.
- You will pay the fee of $22.60 when you arrive at the fingerprinting site.
- You will receive additional instructions on how to receive your results after completing the fingerprinting.
- Results are not automatically sent to the school district. Applicants must present their UEID number (as shown on the receipt provided after fingerprint capture) to the school district for report access.
- ACT 34 PA State Police clearance - https://epatch.state.pa.us
- In completing the PA State Police clearance, the “reason” is employment
- Results are not automatically sent to the school district. Please submit your results to the Athletic Director.
- ACT 151 PA Child Abuse Department of Human Services clearance - https://www.compass.state.pa.us/cwis/public/home
- Once at this site create an individual account
- In completing the Child Abuse History clearance, under the application purpose, check the box titled, “School Employee Governed by Public School Code”
- Results are NOT automatically sent to the school district. Please submit your results to the Athletic Director.
- ACT 126 Mandated Reporter Training
- ACT 24 Arrest/Conviction Form
- ACT 168 Sexual Misconduct/Abuse Disclosure Form
- TB Test:
- The Pennsylvania Public School Code, Section 1418(b) requires that school personnel be tested for TB. School personnel to be tested include employees and independent contractors who provide direct services to students on behalf of a school, including but not limited to teachers, nurses, administrators, physicians, dentists, dental hygienists, custodians, janitors, cooks, cafeteria workers, bus drivers, librarians, psychologists, audiologists and counselors, and student teachers, and volunteers. The Department of Health defines "direct services" as those persons in direct contact with students 10 or more hours/week. Further information regarding TB Tests can be found under School Staff Requirements.
VOLUNTEER COACHES
All volunteer coaches must complete the following clearances and forms in order to be considered for a coaching opportunity:
- ACT 151 PA Child Abuse Department of Human Services clearance for Volunteers
- Fee: FREE to volunteers once within a 5 year period.
- Login or Create an Individual Account to apply online using the Child Welfare Portal. You will be asked to create a Keystone ID username. Be sure to store your Keystone ID and password in a safe place for future use.
- Applicants will receive an email notification with a link. Please print and save the results to a PDF for your records.
- The CWIS Support Center can be reached at 1-877-343-0494.
- Results are NOT automatically sent to the school district. Please submit your results to the Athletic Director.
- ACT 34 PA State Police clearance check for Volunteers
- Fee: FREE to volunteers once within a 5 year period.
- Visit PSP P.A.T.C.H. (Please do not use the Safari web browser when requesting a PATCH check. The Safari web browser is not supported by PATCH and the request will not be completed.)
- Click "New Record Check (Volunteers Only)", check the Volunteer Acknowledgement Section at the bottom and click "Accept". Please identify the Moon Area School District as the organization for which you will be volunteering. Please enter the following phone number: 412-264-9440
- Follow the instructions, then please print and save your results to a PDF for your records.
- If mailing a paper version of the application, please complete the following steps:
- Visit PSP - PA State Police Record Check.
- Go to the "Help" option on the top of the page
- Hover over the "Help" option and a drop down menu will appear
- Scroll down and click on the form that you want to review and print

- Results are not automatically sent to the school district. Please submit your results to the Athletic Director.
- ACT 114 FBI Clearance/Volunteer Affidavit for Volunteers
- Individuals who have resided in the state of Pennsylvania for 10 or more consecutive years may submit a Volunteer Clearance Affidavit in lieu of seeking FBI Criminal History Record fingerprints. This Volunteer Affidavit has been approved by our School Board, and it is the only version that we are permitted to accept. Please submit your results to the Athletic Director upon receipt.
- If you have not been a PA resident for 10 or more years, you must be fingerprinted to volunteer.
- Fee: $23.25 - Payment is made by credit or debit card.
- Applicants must register online with IdentoGO before visiting a fingerprint center. All applicants must pre-enroll online, or over the phone @ 1.844.321.2101, select option 2 to schedule your appointment.
- When registering, you will need to provide the following Service Code: 1KG6XN
- Important: The correct Service Code must be used so the background check is processed.
- Fingerprint center locations can be found on the IdentoGO website. The Beaver Valley Intermediate Unit 27 has agreed to be an IdentoGo site for fingerprinting applicants.
- Applicants will receive an email notification with a link. Please note the link is available only for ONE-TIME access only, and once accessed will no longer be valid. You should only access this link from a device that will allow you to download, print, or save your results at that time.
- Results are not automatically sent to the school district. Applicants must present their UEID number (as shown on the receipt provided after fingerprint capture) to the school district for report access.
- ACT 126 Mandated Reporter Training
- As a Moon Area School District volunteer, the district requires Mandated Reporter training to be completed.(FREE) Recognizing and Reporting Child Abuse: Mandated and Permissive Reporting Training
- This training takes approximately three (3) hours to complete, and is designed to allow you the option of competing the training over multiple periods of time.
- To register for this free training, click the link above and follow these directions:
- Login or click the "Don't have an account" button
- Submit your information, and you will then receive and email with your username and password
- Enter your username and password and click "Login"
- Click "Access Your Course"
- Click "here to launch the course"
- If you do not see the course in a new window, a popup blocker may be preventing it from opening
- Follow the directions to complete the course
- Print your certificate of completion
- Please provide a copy of the certificate to the Athletic Director.
PA DEPARTMENT OF EDUCATION REQUIRED COACHING EDUCATION
ALL COACHES (paid or volunteer) must complete a course under each heading, annually (on or after July 1st), prior to holding practice with their students:
